Athlete Forms & Volunteer Forms-
Getting started!
Athlete Release/MedicalParticipation in Special Olympics Virginia requires athletes to obtain a physical from a medical professional and fill out the Athlete Medical Form and Athlete Release Form every three years. If an athlete does not sign for themselves then a Parent/Guardian Release Form must be completed. Please return to classroom teacher/local coordinator.

Be a Volunteer Part 1Adult volunteers are one of the most important parts of Special Olympics. In order to be a registered volunteer, each individual must complete the Class A Adult Form. It must be reviewed and approved by another Class A volunteer.

Be a Volunteer Part 2Thank you for being a part of the Special Olympics family. It is through the caring, concern and talents of all our athletes, families and volunteers that Special Olympics has become one of the most respected sports programs in the world. Your participation in the Protective Behaviors Training Program is just one more way that you demonstrate your dedication.
